We understand that information about you and your health is personal and we are committed to protecting your medical information. We keep a record of the health care services we provide to you. You may ask us to see and copy that record. You may also ask us to correct that record. We will not disclose your record to others unless you direct us to do so, or the law authorizes or compels us to do so.

You may see your records or get more information by calling Release of Information (at the phone number in the sidebar).

To make a request for copies of your medical records, please do one of the following:

  • Call Monday through Friday, 8:30 a.m. – 4:00 p.m.
  • Ask the Admitting Department for a release form so you can complete it.
  • Print and complete the Authorization for Release of Information form on this site. Mail, deliver or fax it to us.

There may be a charge for copies of your record if they are for personal use and total more than 10 pages. There is no charge to release records to your health care provider.

It may take up to 5 working days for us to process your request.

Our Commitment to Patient Privacy

Maintaining the privacy of your personal health information is of critical importance to our healthcare team. The information we collect from you is treated with respect and sensitivity. We have developed a strict code of confidentiality which governs how we use your information and under what conditions we may disclose it. Our entire staff, volunteers and all medical professionals receive extensive training in patient confidentiality rules and practices.
