Position Summary / Qualifications:
The LPN will be responsible for the care of a specific group of patients on a shift. The LPN works under the supervision and direction of an RN meeting the needs of the patient and family. Position Summary / Qualifications:

Assesses and evaluates patients and establishes appropriate plan of care in accordance with the physician based on realistic functional treatment goals.  Provides direct patient treatments within the Speech Language Pathologist scope of practice with the goal of restoring function and maximizing patient performance in areas including but not limited to:  Dysphagia (including Modified Barium Swallow studies), language/cognitive disorders, speech/voice disorders. Patients population includes:  adults and geriatrics in acute inpatient, swing bed and outpatient settings.  Observes, records and reports the patient’s response to treatment and any adverse changes in the patient’s condition to the physician and appropriate medical staff.  Instructs and advises patients, families, caregivers and other health team members as appropriate to assist in reaching the goals of the plan of care and maintaining patient confidentiality.  Completes documentation as required and submits per hospital policy.  Attends hospital orientation, meetings, case conferences and in service as requested/required.  Obtains authorization for treatment as required and coordinates the plan of care with the physician, patient and other disciplines through verbal or written communication.  SLP is responsible for remaining current in speech language pathology trends and knowledge.  Ability to organize groups of people toward completion of designated projects, ability to self start projects and ability to communicate ideas well is preferred.

Minimum Qualifications:

• Certification/Licensing/Registration:  current Washington state license in good standing.
• Master’s degree with a major in Speech Pathology from an approved/accredited program.

• Medical setting experience required with two years experience preferred.
